Request for a doctor’s letter

You may have been advised that you need a doctor’s letter for the purposes of travel, employment, visa applications, housing etc. Often, you will need a face to face consultation and a medical examination for this to be appropriately completed. Sometimes, your doctor may be able complete the letter by referring to your medical notes.
