Request a copy of your medical notes

Patients have a legal right to access their medical records under Irish data protection law and GDPR. This is known as a Subject Access Request (SAR). You can request a copy of your GP records at any time by contacting our practice in writing by email or letter. No special form or wording is required, and requests are usually processed free of charge. For confidentiality, we may ask for proof of identity before releasing records.

We aim to respond to all requests within one month. In rare circumstances, small parts of a record may be withheld or redacted if releasing them could cause serious harm or breach another person’s confidentiality, as permitted by law.

You will need to complete a SAR form, you can request one at reception or print the form below. You can also write an official signed request. You don’t have to give a reason for wanting to see your records.
