Request a sick cert for work

This service is for patients of the practice only.

An employer may require a medical certificate if you are out sick for more than 2 or 3 consecutive days.

Initial certification for each new episode of sick leave requires a consultation with the GP. Certificate extension may be requested by submitting the form below. We will contact you if you need to be reviewed by a GP. Your cert will be emailed to you within 2 working days. A charge may be needed to process this request.

Please note if you require a Social Welfare cert you will also need to collect an IB1 form from reception to complete and return to the Department of Social Protection.
