Sick Notes / Social Welfare Certs

Sick Notes for Work: A consultation is always required before a doctor can certify that a patient is unwell and before a sick cert is provided. One medical certificate is usually sufficient to cover the full duration of your illness. We do not issue weekly certs.

If a sick cert needs to be extended for a longer time than initially given you can request a sick cert for work below.

Please note the doctor may need to contact you to discuss this further prior to issuing the cert and a further consultation may be required. 24 hours notice will be required prior to collection and a fee may be charged for this service.

Sick notes for children (creches / school)

Please note that if your child is returning to school/childcare after an illness, they will not require a GP certificate. You can liaise directly with your creche and school for their specific return to school protocol.

Illness Benefit

The Department of Employment Affairs and Social Protection (DEASP) provides illness benefit if you cannot work because you are sick or ill. In order to claim the benefit, you must obtain a medical certificate called a ‘Certificate of Incapacity for Work’ from your GP. The certificate will be submitted electronically through a secure online system by your GP. A separate illness benefit must be submitted by the patient simultaneously - this can be sent as a paper Illness Benefit claim form (IB1) by freepost to the Department of Employment Affairs & Social Protection to open your claim - or can be processed through www.mywelfare.ie.

One illness benefit cert will cover the duration of your illness. Your GP will advise when your illness benefit form is finished and you will typically need to see the doctor again if you need another cert.
